Doctors, Nurses, and Social Workers Provide Monthly Home Medical Services to Mobility-Impaired Elderly Patients
Gwanak-gu (District Mayor Park Jun-hee) is launching the second pilot project of the home-visit medical care service for long-term care, aimed at elderly residents who have difficulty visiting medical institutions due to mobility issues.
The "Long-term Care Home Medical Center Pilot Project" supports elderly long-term care recipients to continue living in their communities by having doctors, nurses, and social workers visit the homes of mobility-impaired seniors at least once a month to provide medical services.
To promote this pilot project, the district signed a business agreement on December 27 last year with two clinics: Jeongdawonuri Clinic (Representative Kim Hye-kyung, located in Sillim-dong) and Yonsei Family Clinic (Director Lee Dong-su, located in Bongcheon-dong).
Both institutions were selected for the Ministry of Health and Welfare’s "Long-term Care Home Medical Center 2nd Pilot Project," which has expanded from one site last year to two sites this year.
Through this agreement, the district will promote the home medical service project to eligible elderly residents, and the home medical centers will be able to provide services such as home visits by medical professionals and nursing care through specialized personnel.
In particular, when social workers accompany home visits by the home medical centers, they will develop care plans tailored to the complex needs of recipients and provide systematic care services and integrated welfare services through community linkage.
Additionally, the two clinics plan to prioritize selecting and continuously managing recipients recognized as long-term care grade 1 or 2 among the service beneficiaries.
Meanwhile, the number of long-term care home medical centers currently operating in Seoul has expanded from seven last year to 19 this year. The operation period for the second pilot project of the home medical centers is until December 31.
