NHIS Expands Types and Reasons for Income Adjustment and Settlement
The National Health Insurance Service announced on the 23rd that, following the revision of the Enforcement Decree of the National Health Insurance Act, the eligible income types and reasons for applying to the "Income-Based Health Insurance Premium Adjustment System (Income Adjustment System)" will be expanded.
The Income Adjustment System is a system introduced and operated since September 2022 that allows payment of health insurance premiums based on current income.
With this revision, the types of income subject to adjustment and settlement will be expanded from the current business income and earned income to six types, including interest income, dividend income, pension income, and other income. Additionally, applications for adjustment and settlement will be possible not only when current income decreases compared to the previous year but also when it increases.
The NHIS explained, "In line with the purpose of the system that reflects current income to impose premiums, all changes such as income decreases or increases will be applied to the premiums," adding, "By expanding the application options, individuals can pay premiums closer to their actual income."
From January next year, the adjusted health insurance premiums will be recalculated in November 2026 when income data for the 2025 tax year is linked from the National Tax Service, resulting in additional charges or refunds. It should be noted that even if only one type of income is adjusted, the settlement will be made based on business, earned, interest, dividend, pension, and other income.

Applications for income adjustment and settlement can be made by mail, fax, or by visiting a nearby branch. For those who have reported business suspension or closure, retirement or dismissal, or a decrease in comprehensive income, applications can also be submitted online without visiting the branch.
