Yeoju City Minimizes Care Gaps for Dementia Patients with Customized Case Management
On August 6, the Yeoju City Dementia Safety Center in Gyeonggi Province announced that it is providing customized case management services to minimize care gaps for dementia patients and to improve their cognitive abilities.
The customized case management program at the Yeoju City Dementia Safety Center aims to address blind spots in dementia patient care. It systematically intervenes in and addresses physical, mental, and social needs and related issues, thereby supporting stable Aging in Place (AIP) within the local community and enhancing quality of life.
The customized case management team consists of dedicated nurses and social workers who carry out the program.
After assessing physical, psychological, social, and environmental needs, final decisions are made through deliberation by the Dementia Project Management Committee. Selected individuals can receive services tailored to their specific needs, as identified during the initial assessment.
Dementia patients residing within the jurisdiction are eligible to apply for case management. Once selected, home visits are conducted to provide comprehensive health management, daily living support, in-home safety management, crisis management, family support, and linkage to community resources, including dementia care. For health management, services include medication guidance, management of problematic behaviors, and programs to promote physical health. Fall prevention education is provided for in-home safety. Families are supported through counseling and communication skills training, and assistance is offered with applications for long-term care grades and customized care services.
