The signing ceremony was attended by Mayor Jeong Jangseon of Pyeongtaek City, city officials, directors and staff from Good Morning Hospital, Park Ae Hospital, and Pyeongtaek St. Mary’s Hospital. The event included the signing of the agreement, a commemorative photo session, and sharing of updates on the integrated care initiative.
Going forward, Pyeongtaek City plans to connect discharged (or soon-to-be-discharged) patients in need of care and medical support with hospitals so they can continue to receive necessary services within the community after discharge. In particular, customized support will be prioritized for citizens registered in Pyeongtaek City who are aged 65 or older and require care and medical assistance to return to daily life after acute treatment.
Currently, the city is promoting an integrated care project for elderly people and individuals with disabilities who have difficulty maintaining daily life and require comprehensive support in healthcare, nursing, and care. The community linkage support for discharged patients is one of the city’s key projects aimed at reducing care gaps within the local community.
Mayor Jeong Jangseon stated, “Discharge is not the end of treatment, but the beginning of community care. With this agreement as a stepping stone, we will further strengthen the Pyeongtaek-style integrated care system in close cooperation with hospitals and the community so that citizens can recover more safely and securely where they live.”
