원본보기 아이콘The meeting, attended by representatives from Pohang City, the Pohang Northern Branch of the National Health Insurance Service, Pohang Medical Center, Semyung Christian Hospital, Pohang St. Mary's Hospital, S Pohang Hospital, and Good Seonrin Hospital, discussed and exchanged opinions on measures to connect discharged patients to the community to prevent readmission and institutionalization of acute patients with high readmission rates and to support their return to home life.
They also agreed that to establish the Pohang-type integrated medical and care model for the elderly, local medical institutions, the National Health Insurance Service, related organizations, and local governments must cooperate with each other.
The city plans to continue holding regular work-sharing meetings and promote free exchange of opinions and mutual interaction to build a meticulous care system and establish a community integrated care promotion system without care gaps.
Kang Hyun-joo, Director of the Welfare Policy Division, stated, “As the community has entered a super-aged society, we will make every effort to build a care infrastructure in cooperation with local medical institutions so that the elderly can live independently and stably, creating a community integrated care model.”
Last December, the city was selected for the Elderly Medical-Care Integrated Support Technology Support Pilot Project (hosted by the Ministry of Health and Welfare) and the Long-term Care Home Medical Center Pilot Project (Clinic at Home) through public contests, and plans to officially implement the Pohang-type medical-care integrated care support project this year.
